Monitor these signals weekly. Any single signal may be noise — two or more together warrant attention.
The account signed up but never completed the core workflow that correlates with retention. A new user who never activated is unlikely to return — and may not be worth retaining. But an account that activated months ago and stopped is a different problem.

The person who championed your product inside the account has stopped logging in. When the primary stakeholder goes quiet, the account typically follows within 2–4 weeks. This is often the earliest detectable signal.

Usage of the account's core workflow has dropped significantly over a sustained period. A 40%+ decline over two weeks is a stronger indicator than a gradual month-long trend. Compare week-over-week, not just month-over-month.

The number of active users in the account has dropped. An account where three people were using the product and now only one remains is showing organizational disengagement — not just an individual changing roles.

An open support ticket or a critical bug report with no resolution. Even if the blocker seems small from your side, if it blocks their workflow, it accelerates churn. The longer it remains open, the more likely they are to leave.

A recent payment failure that has not been resolved. One failed payment is often a precursor to churn — especially for smaller accounts where the payment method is personal rather than corporate.

The renewal date is within 30 days and at least two other signals are present. A declining account approaching a renewal decision without intervention is the most urgent scenario.
